How to get a wig covered by insurance

You may be eligible to have reimbursement for your wig if purchased due to medical reasons. Most insurance companies will cover 80-100% of the cost of a wig or hair piece. Follow the steps below to see how.

Filing your initial claim:

  1. Ask your physician to write you a prescription for a Cranial Hair Prosthesis.

    • Request that your physician writes a supplementary letter stating hair loss and its emotional effect. Remember, it is not only cosmetic, it is also for your emotional well-being!

    • Ensure that the diagnosis codes are specific, and the physician states why the cranial hair prosthesis is a DME (durable medical equipment).

    • Use the Healthcare Common Procedure Coding Systems (HCPCS) codes: S8095 and A9282.

  2. Complete the required insurance claim forms according to your insurance provider

    • Insurance companies will most likely ask for a NPI code. Using the NPI database, search for your physician using the Individual Provider search area. The NPI number is a 10-digit number.

    • Ask your physician to sign off on the insurance claim paperwork.

  3. Submit the custom invoice we provide you for the wig you choose.

    Hair Plus Wigs will create a custom invoice for you, ensuring that there is no mention of a wig. We will specifically state that it is a cranial hair prosthesis along with our Tax ID# for you to submit.

  4. Optional: Submit additional items/evidence to reinforce your claim.

    • Write your own letter describing what personal effects your condition has on you (i.e. self-esteem, grief/depression, anxiety, daily activities, relationships).

    • Include pictures of yourself without hair and without makeup.
